Anyone have any good verbiage for these outlets that break like this. They work but would like some good stuff to put in the report.

I'd just say some of the plastic surrounding the grounding slot at the ____ receptacles was damaged and we recommend replacement for optimal safety.

It's not a big deal now, but since the plastic is damaged I'd expect more to fall off and possibly expose energized parts.
